In the case of fractures during the cementation application, CCDL, Inc. will remake the restoration at 50% of the original fee, plus shipping and other incidentals such as soft tissue models that may be required. (See conditional warranty details)
All Vita YZ, Cerec In-Lab, Lava, Procera, Captek and Prcelain Fused to Metal restorations have a conditional 5 year warranty

Conditional Warranty Details
All warranties are conditional upon the patient receiving regular dental recall appointments from the doctor’s practice and meeting with all of the post care recommendations including mandatory night guard compliance. Any damage due to trauma is excluded from any warranty. Additional restorative procedures (including, but not limited to, endodontic treatment, additional of a partial denture, loss and or failure of teeth, etc) may void any warranty. CCDL, Inc. is not responsible for changes in the condition of teeth to receiving restorations (such as, but not limited to, loss of, or fracturing of abutment teeth prior to, during, or after cementation). This warranty does not apply to any restorations that the lab gave notice to regarding any contraindications for the case. All warranties will be remedied by remake of original class of restoration at no charge. Refunds or credits on account are not included in the warranty of CCDL, Inc. |
Terms and Conditions
All accounts are payable within 30 days of statement date, which is the last working day of each month. Accounts not paid within the stated terms will be subject to a late charge of 2% of the unpaid balance and may be subject to C.O.D. status.
